The short answer: Place it on dry skin with the cooling plates flat, the vents clear, the weight balanced and the setting low to start.

How to use a neck air conditioner in eight steps

1. Identify the cooling type
Check whether your product is fan-only or uses active cooling plates.
A fan-only model moves ambient air. A Peltier model adds direct contact cooling.

2. Inspect it before wearing
Keep the device off your neck during the check.
Look for:

a cracked housing;

a loose or damaged cooling plate;

blocked vents;
-
swelling or a changed shape;

unusual heat or odor; and
-
moisture in or around the charging port.

3. Prepare your hair, skin and clothing
Tie back long hair.
Remove scarves and loose lanyards. Keep necklace chains away from the intakes.
Your neck and the device should be dry.
Move a high collar away from the cooling plates and vents. Do not place a wet towel between the device and your skin.
4. Open the device gently
Use only the adjustment range shown in the manual.
Do not twist the arms in opposite directions. Do not force a rigid frame wider than designed.
Hold both sides. Lower the device around the neck without scraping the skin or catching hair.
5. Center and balance it
Place the controls where you can reach them.
Both sides should carry similar weight. The device should not lean forward or pull backward.
Leave clear space at the front of the throat. Breathing, swallowing and head movement should feel normal.
6. Position the cooling plates
Active-cooling plates work best with even contact.
Place them on the skin areas shown in the manual. Many neck-worn designs use the sides or back of the neck.
Do not place hair, thick fabric or padding under a plate.
If only one edge touches, reposition the device. Do not tighten it until it hurts.
7. Check every intake and outlet

Find the intake vents and the air outlets.
They are not always in the same place.
Move hair, collars, hood strings and backpack straps away from them. Warm exhaust from the hot side of a Peltier system also needs a clear path.
Sony’s current REON guidance, for example, uses different vent covers for collared shirts and T-shirts. The model is different from a U-shaped neck air conditioner, but the principle is useful: blocked exhaust reduces performance.[1]
8. Start low, then test movement
Start at the lowest useful fan and cooling level.
Wait a few minutes. Then adjust one level at a time.
Turn your head left and right. Look down. Walk a short distance.
Stop and readjust if the device shifts, pinches, pulls hair or blocks your hearing.
Which setting should you use?
Use the lowest setting that provides enough comfort.

Maximum mode is not automatically the best mode.
It can increase noise and battery demand. It may also make plate contact uncomfortable.
Why does my neck air conditioner not feel cold?

Check the simple causes first.
The cooling plates are not touching evenly
Hair, a collar or poor fit may leave an air gap.
Remove the device. Clear the contact area. Put it back on without over-tightening.
An intake or hot-side exhaust is blocked
Peltier cooling moves heat to the hot side.
If that heat cannot escape, cold-side performance may fall. Move fabric and hair away from every vent.
You are expecting cold air from a cold plate
Some devices cool the skin through metal plates while their fan airflow stays close to ambient temperature.

The battery is low
Some products reduce output as the battery drops or the electronics warm up.
Check the battery indicator. Follow the model’s charging instructions.
Your skin has adapted to the sensation
A strong cold feeling may fade after several minutes.
Do not keep increasing the setting just to recreate the first sensation. Remove the device and check the skin instead.
Why does warm air come from the device?
Warm exhaust can be normal for an active Peltier device.
The cold plate absorbs heat. The hot side releases that heat plus electrical input.[2]
Warm exhaust should leave through its designed vent. It should not be trapped by a collar or directed uncomfortably onto the skin.
Stop using the device if the housing becomes unusually hot, the fan sounds abnormal or the product shuts down repeatedly.
How to improve neck fan fit

Do not use it for running or strenuous work unless the exact manual permits that activity.
Test the fit with the shirt, jacket and backpack you will actually use. A high collar can block a vent or lift a plate away from the skin.
When should you take a break?
Remove the device periodically during a long session.
Check the contact areas and wipe away sweat as directed by the manual.
Stop immediately for pain, burning, persistent numbness, itching, swelling, blistering or skin discoloration. FDA guidance for cold-contact therapy devices uses these as conservative stop signs, although a consumer neck air conditioner is not the same type of device.[3]
Do not sleep while wearing it unless the exact instructions expressly allow sleep use.[5]

Water resistance varies by model. One current Sony wearable, for example, allows limited sweat and splash exposure but prohibits rain, running water and immersion.[4] Check your own manual rather than copying another product’s limit.
After use

Switch the device off before removing it.
Check the skin and device temperature. Let the product return to normal temperature before charging.
Wipe accessible surfaces as directed. Keep liquid away from vents and ports.
Store the device dry and out of direct sunlight.
